Request Account
Ontario Forensic Mental Health Bed Registry has been developed by the Ministry of Health to meet the needs of various stakeholders to the Forensic Mental Health System. These include Crown Attorneys, who require information on availability of assessment resources in order to assist the court with the creation of appropriate Dispositions and Orders; provincial Forensic Program Directors, who require information on availability of resources throughout the system to assist them with matters they bring before the Ontario Review Board; and the Ministry of Health that requires information about forensic system bed use to assist with system coordination and design.
Different users require different information. Access authorizations are determined by these needs.

To access statistics and perform searches, you must first login using a registered email address and password.

Request a New Account
To access this Bed Registry you must first request a login and password. You will need access to a valid email address to activate your account once you have been approved.     
Login
To login to the site, enter your user name (should be your e-mail address) and the password provided, then click the "Login" button to enter the application.

If you do not have a user name or password yet please use the "Request Account" link located in the main navigation section.


Setup New Account
You must have an active account before logging you can login. Your account consists of an active email address and a password. Accounts are free to setup, but your request for an account is subject to approval.

To apply for an account, click the "Request Account" link. You will be notified via e-mail if your account was approved or denied. If approved, you will receive an assigned user name and password to gain entry to the application.


Account - Locked/Disabled
Locked
After 5 unseccessful attempt to log in with your username, your account will become "locked". This is a security feature built into the application to prevent malicious attempts to gain entry. If your account becomes locked out you may choose one of the following options:
  • If you have tried several times and locked the account out every time, you may be using the wrong password. Click the "Forgotten Password" link under the login section and your password will be e-mailed to you.
  • Send a message to the administrator by using the "Contact" link located at the bottom right of this page. Please choose the subject "Locked or Disabled Account"
Disabled
Accounts can only be disabled by an administrator. If your account has been disabled, please use the "Contact" link located at the bottom right of this page to send a message to the administrator. Please choose the subject "Locked or Disabled Account" and provide a brief description if you feel the account has been disabled in error.
